Dear Patient,
Welcome and thank you for choosing Lakeview Internal Medicine for your health care needs. We have proudly served the Upstate for over 20 years, with an emphasis on quality care. We are a Patient Center Medical Home and we have successfully met CMS clinical quality measures (HEDIS, PQRS, MU, MU2 and MIPS) on a yearly basis.
As internal medicine specialists, we are dedicated to advancing the care of adults from adolescents to the elderly. We have the broad range of skills and experience needed to provide our patients with the medical care they need throughout their lifespan. With an emphasis on preventive medicine, teaching patients how to reduce the risk of illness and careful management of long-term conditions, adults in the Upstate can count on Lakeview Internal Medicine for their healthcare needs.
Please complete the enclosed forms and return them to the receptionist prior to setting your appointment. Also bring with you:
- Medications list you currently use
- Health Insurance cards including (primary policy holder information)
- Photo identification and a Form of Payment (Cash or Check)
Once your new patient appointment has been established, please arrive 30 minutes prior to fill out kiosk questionnaires and consent forms.
